Documentary requirements:

1. Form 138
2. Certificate of good moral character (issued by the high school principal, head of the school last attended or a barangay official in the community where the student resides)
3. Certificate of eligibility for admission to college from the Commission on Higher Education (CHED) in cases where the student-applicant cannot reasonably secure his/her school credentials for reasons beyond his/her control, such as:
   o Closure of the school last attended and
   o Damage of documents due to fire or natural calamities.

Application procedure:

1. Submit required admission credentials (Form 138, Certificate of Good Moral Character for Freshmen; Transcript of Records, Honorable Dismissal, for transfer students) at the Registrar’s Office
2. Fill out the Student Information Sheet (SIS)
3. Apply for and obtain a Student Number before proceeding to the Dean’s Office of his/her college
4. Fill out the Student Enrolment Form at the Office of the Dean of the student’s college
5. Get approval of the class schedule or schedule of subjects to be taken during the term (The class adviser or the college dean must sign the class schedule form)
6. Submit the Student Enrolment Form back to the Registrar’s Office for encoding and assessment of fees based on the approved class schedule or subjects to be taken
7. Present a Discount Form, if any, to avail of the tuition/other fee discount privilege
8. Present Student Enrolment Form at the Bursar’s Office and pay tuition and other fees for the term
9. Obtain a print-out of Registration and Assessment Form (RAF) at the Registrar’s Office and have the RAF signed and stamped “OFFICIALLY ENROLLED.”
