1. Original Form 138
2. Photocopy of Birth Certificate
3. Certificate of Good Moral Character
4. 2 pcs of 2×2 ID pipictures
5. Application Form

Testing Fee: Php 450.00

Application Procedures:

1. Secure a FEU-CAT Application Form from the following:
   - FEU Manila Technology Building Lobby – This is open Monday to Saturday, 8:00 a.m. to 5:00 p.m.
   - Your school’s High School Guidance Offices
   - Downloadable free of charge from the FEU website, www.feu.edu.ph.
2. Fill out the form and submit with two (2) pieces of colored 2×2 ID pictures to the FEU Manila Technology Building Information Booth.
3. Pay the testing fee of P450.
4. Get your receipt and schedule.
5. Take the FEU-CAT on your designated date, time, and venue.
6. Bring your official test receipt/ permit and School ID on your scheduled testing date.

Examination date: every Saturday beginning August 15, 2009, 8:00 a.m. & 1:00 p.m.
