Documentary requirements:

1. Photocopy of available grades in fourth year, duly signed by the Principal or Registrar (with no failing grades in fourth year)
2. Accomplished DBTC Recommendation Forms
   - One (1) from the School Principal;
   - One (1) from the Class Adviser or Guidance Counselor;
3. One (1) 2”x2” ID picture
4. One (1) 1”x1” ID picture

Application fee: Php 500.00
