Documentary requirements:

1. Accomplished application form
2. Original form 138
3. Good moral character testimony by the Principal
4. Two (2) copies of latest picture 1 1/2 × 1 1/2
5. Birth Certificate from National Statictics Office (NSO)

Application fee: Php 396.00

Application procedure:

1. Secure an application form from the Recruitment and Placement Department, SDV Bldg., Room 202.
2. Fill out the form and attach one (1) 1 1/2 × 1 1/2 ID picture (colored/black & white).
3. Pay the entrance examination fee to the Cashier’s Office at the DCT Lanai.
4. Submit the form with picture and receipt for examination to the Counseling and Testing Department, SDV Bldg., Room 303.

Exam schedule:
For the month of August & September – examinations are conducted on the 2nd and 4th Saturdays of the month with the following time schedules:

Registration: 7:30 am – 7:55 am
Exam: 8:00 am – 10:00 am

Registration: 9:30 am – 9:55 am
Exam: 10:00 am – 12:00 nn

Registration: 12:30 pm – 12:55 pm
Exam: 1:00 pm – 3:00 pm

Registration: 2:30 pm – 2:55 pm
Exam: 3:00 pm – 5:00 pm

For the month of October onwards – examinations are conducted daily from Mondays thru Saturdays with the same test schedules. Examinees should bring pencil and ball pen for the exam.

Note: Entrance exam result is valid only for two (2) consecutive semesters.

Admission Requirements For:
B.S. Nursing:

1. Height Requirement: Male – 5’2”, Female – 5”
2. 82% and above 4th Year General Weighted Average
3. No Final Grade Lower than 80% in any Subject

B.S. Accountancy:

1. 85% 4th Year General Weighted Average or its equivalent
